Please be advised that <name> and I have recently divorced. As your file reflects, we were joint owners of the property financed through the above-referenced loan. That property is located at <street address, city and state>. By agreement of the parties and decree of the court, I will henceforth be the sole owner and occupant of this property. I have enclosed a copy of the court's Judgment to this effect. The repayment of this loan is now my sole responsibility.
Accordingly, I would like to remove my former spouse's name from the loan documents for this property. Please forward to me any information and/or applications which must be filled out to accomplish this change. In the meantime, I would appreciate it if you would make a note in your file reflecting the new ownership status of the property.
